One of the most detrimental outcomes for a young adult who has aged out of a Transition program is becoming isolated at home—spending the majority of their time watching TV, playing video games, or scrolling through social media. This program exists to prevent that outcome and to ensure continued growth toward independence.
Our goal is to build upon the skills developed during Transition and continue that momentum in a meaningful, structured, and supportive way. Each participant receives an individualized experience, recognizing that no two young adults have the same strengths, needs, or goals.
The program focuses daily on essential life areas, including employment readiness, money management, budgeting, banking, cooking and meal preparation, understanding healthy choices, and navigating the expectations of adulthood. Participants will also develop self-advocacy skills, build healthy relationships, engage in regular exercise, and strengthen age-appropriate social skills.
In addition, the program emphasizes community involvement through enriching social outings, real-world experiences, and opportunities to build meaningful connections.
Above all, our mission is to help each participant grow in confidence, capability, and independence so they can lead fulfilling and engaged lives.
